What is a support coordinator?

A support Coordinator help participants to:

• Connect to the NDIS and other supports

• Monitor plan budgets and supports

• Build capacity to help participants understand the NDIS and achieve their goals

• Act in participant’s best interest

Here at South West Individual Supports we have an extensive wealth of experience and knowledge of the NDIS. We assist our participants in building their capacity to achieve their NDIS goals. We connect our participants with providers, and community access and help to maximize their NDIS plans.

We provide service to any age group and individualise our approach to suit the needs of each participant.
